血流制限トレーニングと認知機能の関係は
バーベルやダンベル、チューブなどで負荷を与えるストレングストレーニングは高齢者のフレイル予防・治療に効果的であり、筋肉や骨を強くして生活を改善するということはすでに周知されている。
そうしたフィジカル面だけでなく、ストレングストレーニングは認知機能やうつ状態を改善することによりメンタル面でも生活改善に役立つということが明らかになりつつある。
加齢によってIGF-1のレベルが低下するが、IGF-1の低下は神経変性を引き起こしたり、心臓血管系疾患のリスクを増やしたり、認知機能の低下を引き起こしたりする可能性があると言われている。(※1, ※2, ※3, ※4, ※5)
ただし高齢者は骨や関節の問題をかかえていることが多く、一般的なレジスタンストレーニングは直ちに適用しにくいことも多い。また高重量を扱うトレーニングでは血圧の上昇も気にかかるところだ。
しかし軽い重量で効果が見込めるBFRトレーニングであれば、骨や関節への負担は少なく、血圧上昇における心配も少なくなる。
さて、実は断続的な低酸素状態にさらすことで、脳の可塑性が高まり、認知機能が高まるという報告がある。(※6, ※7)
BFRトレーニングは低酸素状態を作り出すわけだが、実際にBFRトレーニングを30%1RMで行ったところ、認知機能が改善したと報告されているのだ。(※8)
低酸素状態によって血管の新生も増えてくると思われる。
さらにBFRトレーニングでは通常のストレングストレーニングに比べ、多くの成長ホルモンやIGF-1の分泌を促すことが分かっている。(※9, ※10, ※11, ※12, ※13)
そして運動によって大脳皮質の活動が活発になることにより、認知機能は高まるとされている。(※14, ※15)
実際にBFRトレーニングによって大脳皮質の活動が活発になったという報告(※16)のほか、大脳皮質運動野における酸素化ヘモグロビンが血流制限なしの場合と比べて高レベルになったという報告もある。(※17)
このようにBFRトレーニングによって認知機能の低下を防いだり、改善を図ったりすることができる可能性は非常に高く、今後の研究展開(※18)が期待される。
